Outreach Nurse Referral

Outreach nurses visit clients who are unable to attend clinic alone or with a parent/guardian/carer with a view to supporting their eventual attendance at a clinic.

We accept referrals for clients aged under 18 or who have a recognised vulnerability and are at risk of unplanned pregnancy or sexually transmitted infections.

To make a referral to the Outreach Nursing team complete all relevant sections of our referral form and email to Snhs.sexualhealthreferral@nhs.net

Referrals to any other sexual health email account or postal referrals may not be actioned in a timely manner.
